What that does is shift the work from you to the people trying to
answer the questions.  Personally, I have no intention of reading
through and debugging your entire config.  You can do the work by
showing us the relevant stuff.

If you mean to put the header in the directory that will be indexed,
you should be using
HeaderName HEADER.html
