256? That's not so many...

A busy server I look after has this (and I'm not promising that this is
"well configured"!):

<IfModule worker.c>
ServerLimit 80
StartServers       10 
MaxClients         2000
MinSpareThreads     100
MaxSpareThreads     200
ThreadsPerChild     25
MaxRequestsPerChild  0
</IfModule>

I'm sure it can go much higher...

If on unix you will need to make sure that you set the nofile parameter
(possibly in /etc/security/limits.conf)

Something like:

www hard nofile 16384
www soft nofile 16384
