I had the same issue after update to Apache/2.0.48 (Win32).  All double spaces and ' were turned
into ? marks.  

I changed the httpd.conf from:

AddDefaultCharset ISO-8859-1

To: 

AddDefaultCharset UTF-8

I tried stopping and restarting apache on my win2K box but it wasn't until 
the server rebooted last night and they're gone now.

My old server didn't have this directive however the new one was set to 
UTF-8 as default. I changed it to "AddDefaultCharset On" and all is OK 
now. I have restarted the server and pages now come up correctly.
