I think it is related to browsers and not servers..

I've seen various results by having just any icon filename linked through 
the tag or by using exactly "favicon.ico"..  I've seen alternate results by 
having that icon in the root as opposed to specified in some path..  and 
furthermore have seen other results before and after the user has saved the 
page as a favorite..

I think you can get what you are looking for on the MSDN or some forum 
therein.
