This isn't limited to Apache. I'm seeing the same thing happen with
Iplanet 4.1SP12.

As a previous response to my question, someone suggested changing the
'KeepAlives On" setting to 'KeepAlives Off" in httpd.conf.  I did this,
and the number of failed POST submissions went from about 20 per hour to
about 2 or 3.  Also,  in the remaining few failures per hour, I've seen
it happen to non-IE browsers, including Netscape, Opera, Safari, etc.
So changing "KeepAlives" did fix most of the failures, but not all.
