in answer to your question:
the message in  error_log under /etc/httpd/logs says :

[Tue Jan 13 09:39:07 2004] [error] [client 192.168.0.111] (13)Permission 
denied: access to /filename denied

my DocumentRoot is different from  / .
I wonder : shouldn't it give the full path instead of /filename ?
for example" if DocumentRoot is /usr/local/test , for example, shouldn't it 
give
/usr/local/test/filename insead of /filename?
