It turns out that IE6 has a control under Tools, Internet Options, Security 
(for Internet), Custom Level, scroll down to User Authentication, select 
logon: "Prompt for user name and password".  Apparently IE6 will not 
respond to the <401 Authorization Required> response output from Apache 
unless this option is selected.

Wow, what a lot of work for a curious default setting in IE!  Thanks to all 
for the help
