Did you happen to upgrade apache but not recompile the .so?

A bus error (and its cousin, the segmentation fault) is a fairly low
level problem - it means that something is trying to address a
non-existent location on the PCI bus. This sort of problem can occur
because of inconsistent compilations in shared objects. For example, if
you compile mod_foo against apache 1.3.27 and then upgrade apache to
1.3.28, your old mod_foo might not work any more... The solution is to
recompile any loadable code (i.e. mod.foo.so) against the new apache.

We are using apache w/mod_proxy as a reverse proxy.  Everytime we have
an outage on our main core network switch we receive the following
errors in the apache error_log and the web server will no longer proxy
until we restart apache.  Our apache web server will still serve up
local content which makes me think it is a proxy module issue.  All
effected backend servers are hanging off of the switch that went down.
Even though this does not happen often I still donot think a network
outage should cause a proxy server failure.  I have monitored the amount
of httpd processes being used during this time and it is only peaking at
160 and we have set the MaxClients to 256.
