You could use mod_rewrite to rewrite all requests to a particular
perl/php/cgi script, with the original request as an argument.  This
script could do whatever you want in terms of including or modifying the
content.  However, this would mean the pages are no longer static (and
wouldn't be cached as well) and there would be some CPU overhead of
course.
