Sorry guys, but I'm a real noob.

I've been hunting around the Redhat 9 filesystem all morning.  Where is the
httpd binary?  Where is the mysql binary?   In other words, I have to
configure mod_auth_mysql with the --with-apache=dir and --with-mysql-dir=
directives.  What do I put in for these?  This is an out of the box install
with no custom installs...

Sorry I'm so hopelessly stupid...

Right - you need to tell the module how to connect to mysql.
