The file-path is relative to ServerRoot. Is it where the password resides?
If not, you need to give the full path. See:

On another note, it isn't wise to put password files in web space.

Why do you have the <Limit>-block here? See:

Where in the configuration file? Does it apply to the directory where the
access file resides? Also check, and doublecheck that there isn't another
one that overrides it.
