okay Josh, here's what I have just done (yet again):
I've commented out everything _except_
RewriteEngine on
in the .htaccess file

and this is the output to the log file (per your request):
 - - [22/Jun/2003:17:57:27 -0400] "GET /index.html HTTP/1.1" 500 546 "-" 
"Mozilla/5.0 (X11; U; Linux i686; en-US; rv:1.2.1) Gecko/20021204"

I don't understand - you mean send headers out? yuk. anyway, not possible in 
this situation.

NO rewrite rule is going to work while RewriteEngine on fails
