There is no simple way to do this... Apache just gets files and serves them. If you want to
open them and edit the contents, you need to run some kind of server-sided application to
do this. If you think about it, it is really complicated to inject something like an <img>
tag into an arbitrary HTML page - you need to do some careful pattern matching to make sure
you are not breaking any mark-up in the page. The final appearance would be quite unpredictable...

One workaround might be to serve up all content inside a frameset and have your banner in
the top frame. But that would be really horrible website :-(
