A big "Thank You" goes out to Jason Carlson for this.  Apparently you need
to have the LoadModule for mod_throttle at the end of your other
LoadModule statements, and the AddModule for mod_throttle at the end of
your other AddModule statements.  My policies are working perfectly now.
