I figured out how to fix it. To fix it, I changed this line:

LogFormat "%v %h %l %u %t \"%r\" %>s %b \"%{Referer}i\" \"%{User-Agent}i\""
combined

to:

LogFormat "%V %h %l %u %t \"%r\" %>s %b \"%{Referer}i\" \"%{User-Agent}i\""
combined

Notice the first formatting arguement.
