Wonder if it could have anything to do with these. Although I see no
documentation that says it needs them.

#LoadModule proxy_module modules/mod_proxy.so
#LoadModule proxy_connect_module modules/mod_proxy_connect.so
#LoadModule proxy_http_module modules/mod_proxy_http.so
#LoadModule proxy_ftp_module modules/mod_proxy_ftp.so

Change the Order value from deny,allow to allow,deny.
I don't see any other information to help you with the rest of it.
