See:
http://httpd.apache.org/docs-2.0/logs.html#combined

Add the following:
  LogFormat "%h %l %u %t \"%r\" %>s %b \"%{Referer}i\" 
\"%{User-agent}i\"" combined

and replace
  CustomLog logs/access_log combined
with
  CustomLog logs/access_log combined
