You can run the script using `php -q`, and output the cgi headers within the
script itself.  php, by default, prints:

X-Powered-By: PHP/4.2.2
Content-type: text/html

PHP can also be used as a binary using the SetHandler (IIRC) directive, with
no special cgi tricks.
