I played with this some more and there does seem to either be a bug or
some lack in the documentation for mod_include.
This "if" worked:
<!--#if expr="$REDIRECT_URL = '/test/'" -->
This "if" didn't:
<!--#if expr="$REDIRECT_URL = /test/" -->
