I just traced an issue in 1.3.23 in which a 500 (Internal Server
Error) page can be returned without any entry in the error log
file.  It occurs under some set of circumstances involving
Satisfy Any, at least, if not others.

Do you have any mixtures of Allow/Deny Auth*File, and Satisfy in
the scope that includes the location in question?
