You are not providing much detail.  What is in the access, error, and most
importantly, the rewrite log file?  What happens in the browser.

I've never tried this myself, but one guess is

RewriteRule ^/(.*) http://127.0.0.1/$1?ra=%{REMOTE_ADDR} [P,L]
