Generally at least 10's to 100's of times a second, if not much much more.

And only if the file system caching and general kernel cahcing is not
sufficient.  Which makes it even more of a rare thing to need it.

It is a directive to be careful with - as it is easy to have negative side
effects due to increased footprint of resources such as memory, fd's etc,
down up to kernel land.
