Subject [PATCH] make CacheEnable perdir

This is an ugly patch. Here's what is basically does.

-CacheEnable takes only one argument -- Off to disable, or the name of a 
provider

-save filter is now set in fixup.  Provider may be overriden by 
environment.  TOD0: see if env sets off.

-don't use ap_provider stuff.  No way to get a list of providers.

-most cmds are now per dir, except for ignoreheaders and 
ignorecachecontrol which are still per server.

-eliminated ap_cache_get_providers.  Each provider gets a shot at 
serving (create_entity)

Lots of this can probably be done better, but this is a good first attempt.
