Subject [PATCH] new include in mod_include.h

mod_include.h requires util_filter.h, but it doesn't include it.  This is
fine for Apache itself, because everywhere that mod_include is used,
util_filter is already included.  However, for people extending
mod_include, this means that they must include util_filter.  Here is a
patch to fix it.
