Please test and continue to review.

All of the changes that I have meant to go in are now committed.

Outstanding issues:
- Switch ap_http_filter to use ap_get_brigade rather than ap_getline
  (requires ap_brigade_to_buffer function that I had posted previously)
- Call get_mime_headers again when we receive the zero chunk so that we
  read the trailer headers correctly.  This requires refactoring
  get_mime_headers, so I'd rather wait a bit on this.
- Don't actually use a socket bucket.  Both Ryan and Greg have
  suggested this.  I'm not really sure what it buys us, but I'll take
  a look at this.  Shouldn't be hard for someone else to pick up if
  they want to do this.

None of these are showstoppers, IMHO.
