SAFE_MUTEX results in a dual-level macro expansion... the first
for accept_mutex_child_init (which needs to check for a NULL
pointer before calling) and then the handling of single listen.

I'm wondering if it's actually in the accept_mutex_child_init 
phase where it dies. I'll look...
