Having had a chance to investigate what is actually happening, it looks
like we are spawning child processes wildly.  What bothers me about this,
is that I have been able to reproduce this with both the prefork and
mpmt_pthread MPMs, which means this problem could be very far reaching.  I
plan to investigate this over the next few hours/days until this is
solved.
