Subject apr_hdtr_t without sendfile.

Okay,  I have had a chance to look at the build break with sendfile.  I
dislike having apr_hdtr_t not defined on platforms without
sendfile.  IMHO, apr_hdtr_t is a useful type, even without sendfile.  This
would also make the fallback case very clean, because we would call:


writev on the headers
loop while data in file
    read from file
    write data
end loop
writev on the trailers.

This is reasonably clean IMO.

I'm going to code this now, and test it as best I can, but I would
appreciate it if others would test it too, most notable those people who
are on platforms without sendfile.

P.S.:  I have more time now, and I plan to hack Apache more often again,
