Subject where/why does MULTITHREAD get enabled?

assertions: 

  It is not enabled based on config but is turned
  on unconditionally for Win32.  

  It only has meaning for Win32.

  mpmt_pthread/scoreboard.c and prefork.c have conditional
  logic based on MULTITHREAD but this is a hold-over from
  1.3 days where Win32 used some of the code that ended up
  in those MPMs.

  It is no longer of interest and in fact is misleading because
  most 2.0 multithread configurations shouldn't have MULTITHREAD
  defined.
