Well, you saw the size of the patch. That was the main reason; it was
just too simple to resist doing. Plus, the pthread MPM has some cool
features integrated that the prefork MPM doesn't, like elimination of
signals for graceful restart. The fewer MPMs that have to be
supported, the better. And, unless/until the Group officially changes
its position, the hybrid server in the apache-apr repository is still
destined to be 2.0, so it needs to support the prefork model.

You'll be more interested in the other MPM I'm working on (which I'll
commit the first stages of soon. I've taken the pthread MPM, set the
number of processes to be fixed, and almost finished eliminating the
shared memory scoreboard (though it doesn't have any way of talking to
any future mod_status yet). After that, thread creation & deletion
comes next.
