Subject nmake clean

Have you noticed how a make clean on Win32 MSVC makefiles doesn't actually
clean very well? It leaves the intermediate build directories (ApacheD,
etc), files .ncb, .opt, .plg, .pch, .ilk and all sorts of other rubbish.
Not to mention .dsw which it insists on creating. I think we should clean
these things out with "make clean" in makefile.nt, or at least provide a
new target "make properclean" which cleans them away.
