Additional change:
For mod_info and mod_autoindex, the server name and port appear in the
<TITLE> for the request.

The "Signature" is usually a <HR>, followed by
  <ADDRESS>@@SERVER_BASEVERSION@@ at <A HREF="mailto:@@ServerAdmin@@">@@ServerName@@</A>
Port @@Port@@</ADDRESS>
where the @@xxx@@ are replaced by their respective values.
