One note: If you are going to document this, please mention that it only
applies to files sent by the core (no includes, CGI scripts, or anything
sent by any other module), and only when byte ranges aren't being
requested. As it is now, the docs give a false sense of security, by
implying this directive adds a Content-MD5 header for every response.
