Some clients of ours had created NCSA-format imagemap files
that had incorrect default lines:

    default
    rect home.html 0,0 325,78
    rect 06.2.html 190,804 308,842
    rect 07.html 88,768 187,801

When clicking in a non-defined region, the server spit back a
"internal server error" sort of message to the user, which
was about the best it could do.  However, it failed to
add anything to the error_log file where I would
have noticed the problem and been able to address it.
