Subject: ScanHTMLTitles misses <title> directives
Affects: mod_dir.c
ChangeLog: Make check against <TITLE> case-insensitive
Comments: Since HTML directives are case-insensitive, and use of
lowercase tags is fairly common, ScanHTMLTitles should support that.
