I'd be more tempted to say browser bug or proxy bug somewhere out there.
If you look at http://bianca.com/, you'll see it references images on
images.bianca.com and www.apache.org.  Anyone want to set up the test
case?  A page with two <IMG> tags going to distinct hosts and look at what
various clients stick in the Host: header for the second tag.
