Um, easiest way to do this would be to use the 

LogFormat "%h %l %u %t \"%r\" %s %b \"%{Referer}i\" \"%{User-Agent}i\""

BTW, the docs state:
%...{Foobar}i:  The contents of Foobar: header line(s) in the request
		sent to the client.

Shouldn't that be "... in the request sent to the server."?
