I have a test case that demonstrates the problem every time.  We have a
separate server on an internal host on a non-standard port that is
protected by username and password. Fire up Netscape 2.0, ask for
"http://something.ukweb.com:someport/" and get your password wrong and it
loops forever.  Fire up Netscape 2.0, ask for
"http://something.ukweb.com:someport/somedir/" and get your password wrong
and it works normally.  Hmmmm.
