Read buffer size and flush threshold tuning

Allows to configure ReadBufferSize, FlushMinThreshold, FlushMaxThreshold and
FlushMaxPipelined (instead of hard coded values).

For your proxy_connect case where large files may be rsync-ed, something like
the below looks not absurd:

# Read more at once than the default 8K
ReadBufferSize 65536
# Don't block below this (i.e. more than ReadBufferSize)
FlushMaxThreshold 131072
# Always flush (non-blocking) when asked to write (i.e. don't retain data)
FlushMinThreshold 1
