Fast question, exactly which version of Apache 2.4?  There has been a lot of
activity on mod_headers.

You may want to obtain the latest source from
http://svn.apache.org/repos/asf/httpd/httpd/branches/2.4.x/modules/metadata/mod_headers.c
and compile this using apxs.  Simply load the replacement module in place of
the old module by stopping and then starting your server instance again, and
see if your observed behavior persists.

http://svn.apache.org/viewvc/httpd/httpd/branches/2.4.x/modules/metadata/mod_headers.c
gives you an idea of what all has changed in that module over the past few
years.
