Consider following rules:

RewriteEngine On
RewriteBase /
RewriteRule ^(.*/)?index\.php$ $1 [R=301,L]
RewriteRule ^(.*)$ index.php?page=$1 [L]

These rules results in infinite loop, where someurl.com/?page= redirects itself
to the same url. It will be good to add additional flag to break this loop,
because [L] does not do it - mod_rewrite starts processing ruleset from
beginning.

BTW, I know that in many cases it is possible to use RewriteCond to avoid it,
but sometimes it is really tricky.
