After some further investigation, I've discovered that I wasn't seeing the
userid or REMOTE_USER because I was looking at a restricted context (my proxied
server is Tomcat, and I was running a CGI script under Tomcat). If I inspect the
headers directly from my server (e.g., from a Tomcat JSP page), I see that these
headers ARE being forwarded, but the value is, indeed, wrong.
