patch to fix Via: behavior when ServerName not set ot set or UseCanonicalName
Off

This also fix the :port appending behavior.
proxy_http.c was calling ap_get_server_name (resp. ap_get_server_port) to fill
Via: HTTP header, with server name (resp. server port), but these functions
were confident into request_rec informations and were even returning data from
the request : hostname from Host: (resp. Port from uri parsed).

The submitted patch might fix this bug.
