About the patch:

It needs to return OK instead of r->status.  Otherwise, an error is triggered. 
OK will allow r->status to be respected when appropriate.

        if (status == APR_SUCCESS
            || r->status != HTTP_OK
            || c->aborted) {
            return OK; /* r->status will be respected */

When returning r->status, you can get a complete 200 response then a 500 error
document ;)
