SSLMutex file:xxx is non-portable.

SSLMutex default is portable, but doesn't accept an argument.

I've suggested in the past that we allow 'default' to accept a filename
string, to be used or discarded at the will of APR (or even stripped to
make a name or numeric token for those platforms with more cryptic 
semaphore identifiers.)  This is probably the right approach, and I'll
revisit the idea so that we can adopt

  SSLMutex default:logs/ssl.mutex

as the 'stock' convention.
