Fix segmentation fault on stale cache when mod_disk_cache is used

Summary: Fix segmentation fault on stale cache when
                    mod_disk_cache is used
           Product: Apache httpd-2.0
           Version: 2.0.48
          Platform: All
        OS/Version: All
            Status: NEW
          Severity: Normal
          Priority: Other
         Component: mod_cache
        AssignedTo: bugs@httpd.apache.org
        ReportedBy: ast@domdv.de

modules/experimental/cache_storage.c on the one hand allocates a cache
structure without resetting the structure's contents which can lead
to segmentation faults when subsequent functions fail.
Furthermore it always assumes that info->filename is set which is not the
case when mod_disk_cache is used.
This combination leads to segmentation faults. Patch follows.
