It looks to me like this is a documentation error.  What ErrorHeader does
is set r->err_headers_out.  But, a bit misleadingly, this implies that
the header goes on both the normal and error responses.

Although my knowledge in this area is limitted, I don't believe there is any
easy way (either in the configuration or in the code) to specify that a header
will be sent only on error responses.

So my recommendation is to change this to a doc bug.  But I'll leave it
for someone a bit more familiar with this stuff to verify.
