I believe that this may have something to do with the header processing being
handled as a sub-request. Mod_include would see the "<!--#echo
var="DOCUMENT_URI"-->" as being processed from inside the file smth.shtml. All
references to environment would be from that reference point (including
DOCUMENT_URI). Try adding "<!--#printenv -->" into smth.shtml to see what I
mean. What you might be looking for is REQUEST_URI instead of DOCUMENT_URI.
