  Really simple.  Var files work without multiviews, -only- if the client
  requests the var file itself (e.g. index.php.var).

  If you use multiviews, Apache performs a wildcard search on the given
  request name.  If it hits on a .var file, it selects from -all- the mime 
  choices present in the .var file.  If it hits on other files (.en, .fr etc)
  it adds those specific mime choices as well.  Apache then attempts to serve
  the best-match, or depending on the ForceLanguagePriority rules, it will
  deliver an HTTP_MULTIPLE_CHOICES or HTTP_NO_ACCEPTABLE_VARIANT response.

  Because of the wildcard search on the directory, -yes-, multiviews is slower.
  But you aren't asking Apache to serve the named file, you are asking us how
  to find a filename pattern -similar- to the request name (e.g., index.php.var
  -instead- of index.php.)  For that, you must use multiviews.
