mod_include does not recurse

A .html file includes a .ssi file which contains some ssi statements too. 
When using SetOutputFilter INCLUDES everthing works fine. 
When using XBitHack on or full, nothing is done (the .html file has 0750).
When using AddHandler server-parsed or AddOutputFilter INCLUDES for the
extensions html and ssi, only the html file is parsed.

I haven't tested this on recent versions, but it surely has worked in the past.
Are you ABSOLUTELY SURE that both an AddOutputFilter
and an "Options Includes" applies to the .ssi files?
